How do I correct a mistake on my change of address form?

If you discover a mistake on your change of address form once you've already submitted the form, you will need to contact your local post office to correct the problem. At your post office, you will need to request a cancel forwarding order and replace your order. To find out where the closest post office is to your home, you can call 1-800-ASK-USPS (1-800-275-8777).
